It is the responsibility of Carlow County Council to assess the competency of applicants for the roles detailed below under the Safety, Health and Welfare at Work Act, 2005 and the Safety, Health and Welfare at Work ( Construction ) Regulations, 2006.

Failure to answer all questions in Parts 1 to 10 of the questionnaire, may adversely affect the assessment. Carlow County Council reserves the right to seek further information and / or require any person being considered for either of the roles to attend an interview. Please attach or forward any additional documents, which support the responses you have provided in this questionnaire.
Once you have completed the questionnaire, please select ' Print Form ' to retain a hard copy for your records. After you have printed the completed form please select ' Submit by Email ' in order to forward the information directly to Carlow County Council.
